ADDRESS / EMPLOYMENT CHANGES

YOU must notify the Office of th Chapter 13 Trustee and your attorney of all changes to your address, employment, loss of employment or prolonged illness. Notification must be written. Please be sure to print your name, address and case number and sign your request. It is important that we have a current address on file at all times. If we do not have your correct address, you may not receive annual reports or refunds that may be due at the conclusion of your case.
